How to Find Your Radiology Report
Most radiology reports are available in your patient portal within 24–72 hours of your exam. Select your portal below for step-by-step instructions.
MyChart by Epic
Used by: Cleveland Clinic, Mayo Clinic, Kaiser Permanente, Johns Hopkins, NYU Langone, Mass General, UCSF, and hundreds more.

Go to your health system's MyChart website and sign in with your username and password.

In the top navigation, click or look for the section in the left sidebar.

Select . Imaging results may be listed here alongside lab work.

Alternatively, navigate to → → to find the full radiology report as a separate document.

Click on the result you want. The report text will appear — select all the text (Ctrl+A or Cmd+A in the text area), copy it, and paste it into RadDecoder.

Open the MyChart app on your phone and sign in.

Tap the icon (☰) in the top corner, then select .

Tap to see imaging and lab results.

Tap the result you want. The report text will be displayed on screen.

To copy on mobile: press and hold on the report text until the text selection handles appear, then drag to select all the text. Tap Copy, then return to RadDecoder and tap Paste from Clipboard.
FollowMyHealth
Used by: Allscripts/Veradigm health systems, many community hospitals, physician practices, and specialty clinics across the US.

Go to followmyhealth.com and sign in. You can also sign in using your Google, Facebook, or Apple account if you set that up when registering.

From your dashboard, click in the top navigation bar.

Select from the dropdown menu. Radiology results are listed here alongside lab results.

Click on an imaging result to expand it. The report text will appear in the detail panel on the right or in a modal window.

Select all the report text, copy it, and paste it into RadDecoder. If the report opens as a PDF, use your browser's PDF viewer to select and copy the text.

Open the FollowMyHealth app and sign in.

Tap at the bottom of the screen.

Tap and scroll to find your imaging study.

Tap the result to open it. The report narrative will be displayed as text.

Press and hold on the report text to select it, drag to highlight the full report, then tap Copy. Return to RadDecoder and tap Paste from Clipboard.
Oracle Health (Cerner)
Used by: Ascension Health, CommonSpirit, Intermountain Health, Children's Hospital networks, many VA facilities, and large regional health systems.

Sign in to your health system's patient portal. Oracle Health portals are often branded under the health system's own name (e.g., "MyAscension," "MyCare") rather than the Cerner/Oracle name.

Navigate to the or section.

Select or . Radiology results are typically listed here.

Click on an imaging result. The radiologist's report will be displayed — some portals show a summary first with a or link to access the complete text.

Select all the report text, copy it, and paste it into RadDecoder.

Open your health system's patient portal app. Oracle Health/Cerner-powered apps are usually branded under the health system's name.

Tap or (the label varies) in the bottom navigation.

Tap or and scroll to find your imaging study by date.

Tap the result to open the full report text.

Press and hold to select the text, copy it, then return to RadDecoder and use Paste from Clipboard.
Tips for Copying Your Report
Desktop — Select All
Click anywhere in the report text area, then press Ctrl+A (Windows) or Cmd+A (Mac) to select everything, then Ctrl+C / Cmd+C to copy.
Desktop — Print to PDF
If the report is hard to select, use your browser's Print function (Ctrl+P) and choose "Save as PDF." Open the PDF, select all text, and copy.
Mobile — Text Selection
Press and hold on the text until handles appear. Drag the handles to cover the full report. Tap Copy, then use Paste from Clipboard in RadDecoder.
What to Include
Copy everything — header, clinical indication, technique, findings, and impression. RadDecoder uses the full report for the best explanation. Personal info is automatically removed before processing.
Don't see your portal listed?
Your health system may use a different portal or a custom-branded version of one of the above. In general, look for sections labeled Test Results, Diagnostic Reports, Imaging, or Documents after signing in. If you're unable to find your report online, contact your health system's medical records department or call the radiology department directly — you are entitled to a copy of your records.
